New Patients are Always Welcome at Yubadocs

No matter where you live, we'd love to have you as a patient. Keep in mind that a patients' first visit generally takes longer, and is slightly more expensive than successive visits, because initial information must be recorded.  If you have not been seen here in more than 3 years, your visit will be considered a new visit as all your information has to be updated.

If you would like to decrease the waiting time of your first visit, and you have a fax machine, please print, fill out, and fax this form to (530) 274-7679.

If you do not fax us the form in advance, you can simply bring the completed form with you when you come in or fill it out at our office.
